Health Insurance

Lakeland's health insurance plan provides eligible employees and their eligible dependents access to medical, dental, and vision care insurance benefits. Employees in the following employment classifications are eligible to participate in the health insurance plan:

* Regular full-time employees

* Regular part-time employees (the cost to the employee will be pro-rated according to hours scheduled to work per week)

* Introductory employees who, upon successful completion of the introductory period, will become regular full-time or regular part-time employees

Eligible employees may participate in the health insurance plan, subject to all terms and conditions of the agreement between Lakeland and the insurance carrier.

Details of the health insurance plan are described in the Summary Plan Description (SPD). An SPD and information on cost of coverage will be provided in advance of enrollment to eligible employees. Contact the Administrative Services Manager for more information about health insurance benefits.
